Medical certificate Form 1A: when you need it, who signs it and how to submit it

By , Editor and reviewer · Updated · 15 min read · Independent guide, not a government website

In short

Medical certificate Form 1A is the doctor-signed fitness certificate under the Central Motor Vehicles Rules. A registered medical practitioner fills it after examining you in person. It is needed for transport vehicle licences and for renewals at age 40 or above; most private-vehicle applicants under 40 give only a Form 1 self-declaration. A Form 1A stays valid for one year from issue.

Key takeaways

  • Form 1A is signed by a registered medical practitioner; Form 1 is a self-declaration you sign yourself.
  • Transport vehicle licences always need Form 1A, and Section 15(1) asks for a medical certificate with renewals at age 40 or above.
  • A Form 1A certificate is valid for one year from the date of issue and carries your passport-size photograph.
  • Form 1A travels with the main application: Form 2 for a learner licence, Form 4 for a driving licence, Form 9 for renewal.
  • The RTO charges no separate fee for Form 1A; any charge is set by the doctor or hospital.

What is medical certificate Form 1A?

Medical certificate Form 1A is the official format in which a registered doctor certifies that a licence applicant is physically fit to drive. It comes from Rule 5 of the Central Motor Vehicles Rules, 1989, and the licensing authority reads it alongside your learner, driving licence or renewal application before deciding.

Five steps to get a medical certificate Form 1A signed by a registered doctor
The doctor completes the certificate only after seeing you in person.

Where the name comes from

The rules number every licence form. Form 1 is the self-declaration, Form 1A is the doctor’s certificate that sits next to it, and Form 2 is the learner licence application. People shorten it to medical form 1A or form no 1A.

What the RTO does with it

The licensing officer checks that the certificate is recent, signed by a registered doctor and matches your identity. A clean Form 1A lets the application move to the test or approval stage.

Form 1 A medical certificate: what does the rule say?

The rule behind the Form 1 A medical certificate is short. Rule 5(1) says most applications carry a self-declaration in Form 1, while an application for a transport vehicle licence needs a medical certificate in Form 1A from a registered medical practitioner. Rule 5(2) adds validity and photograph conditions.

The Act behind the rule

Section 8(3) of the Motor Vehicles Act lets the Central Government prescribe the certificate, and Section 15(1) asks for one with renewals by holders aged forty or above.

Why the wording matters

Because the requirement depends on vehicle class and age, two people at the same RTO counter can be asked for different forms. Read your own case, not a friend’s.

Who needs medical form 1A?

You need medical form 1A if you apply for or renew a licence for a transport vehicle, such as a taxi, bus or goods carrier, or if you renew any licence at age 40 or above. Most first-time private car and scooter applicants under 40 only give Form 1.

Common cases that need it

  • A commercial driver adding a transport class.
  • A 45-year-old renewing a car licence.
  • A bus or truck driver renewing a transport licence at any age.

Cases that usually do not

A 19-year-old applying for a learner licence for a motorcycle, or a 30-year-old adding a car class to a scooter licence, normally signs Form 1 instead.

Is Form 1A needed for a private car or bike licence under 40?

Usually not. For a private, non-transport vehicle, the proviso to Section 8(3) says no medical certificate is required, and Rule 5(1) accepts a Form 1 self-declaration. That covers most learner licences for cars, scooters and motorcycles taken by people under 40.

When the RTO may still ask

Section 9(3) lets the licensing authority ask a driving licence applicant for a medical certificate. If your declaration mentions a condition, expect the officer to want a doctor’s view.

Being honest on Form 1

Ticking “no” for a condition you have can lead to the licence being questioned later, especially after an accident. Declare it and let the doctor and RTO decide.

When is Form 1A for DL renewal required?

Form 1A for DL renewal is required when the holder is 40 or older, or when the licence includes a transport class. Section 15(1) of the Act asks for a medical certificate with those renewals. Under 40 with only private classes, a Form 1 declaration normally goes with the renewal.

Renewal at 40 and after

Licences issued between 30 and 50 last 10 years, so many people meet Form 1A for the first time at their first renewal after turning 40. Our DL renewal guide explains the full renewal steps and fees.

Transport licences

Commercial licences need a fresh Form 1A at every renewal, whatever the age, because of the stricter rule for transport vehicles.

Form 1A for driving license applications: learner, new licence and added class

Form 1A for driving license applications appears in three places. Rule 10 lists it with the learner licence application, Rule 14 lists it with the driving licence application, and Rule 5 covers an added class. Each time the phrase is “where required”, so it depends on vehicle class and age.

Learner licence stage

If your learner licence is for a transport vehicle, the Form 1A goes with Form 2 and three photographs.

Permanent licence stage

Rule 14 asks for Form 1A with the driving licence Form 4 where required, along with your learner licence, photographs and proof of age and citizenship.

Added class

Adding a transport class to a private licence brings in Form 1A even if your first licence only needed Form 1.

What is Form 1 driving license self-declaration?

Form 1 driving license self-declaration is a one-page statement in which you confirm, in your own words and signature, that you have no condition that stops you driving safely. No doctor signs it. Rule 5(1) makes it the default fitness document for private-vehicle learner, driving licence and renewal applications.

Your own signature covers Form 1; Form 1A always needs a doctor.

Who signs it

You do. On an online application the declaration is often built into the form, so you confirm it on screen before payment.

What it asks about

It asks you to declare conditions that could affect driving. Read the questions on the current form rather than relying on an old copy, because state portals update their versions.

How to fill the self declaration form for driving licence?

Fill the self declaration form for driving licence in the same name as your ID, answer every question honestly, and sign and date it. If you use glasses or have any listed condition, say so. A wrong answer can delay or later cancel the licence.

Simple filling tips

  • Write your name exactly as on Aadhaar or your age proof.
  • Do not leave questions blank; write “no” where it applies.
  • Sign in the same style as on your application.

If you answer “yes”

Answering yes is not an automatic refusal. The RTO may ask for a Form 1A from a doctor who can comment on the condition.

Medical form 1 vs Form 1A: what is the difference?

The difference between medical form 1 and Form 1A is who vouches for your fitness. In Form 1 you vouch for yourself. In Form 1A a registered medical practitioner examines you and certifies the result. Form 1A carries your photograph and lasts one year.

Why both exist

The two-level system keeps routine private licences quick while giving the RTO a doctor’s opinion where the risk is higher.

A common mix-up

Some people search “form 1 medical certificate” when they mean Form 1A. If a doctor’s signature is needed, it is always Form 1A.

Medical certificate Form 1A PDF download: where is the official copy?

For a medical certificate form 1A PDF download, use the Sarathi portal of the Ministry of Road Transport and Highways or your state transport department’s website. Your local RTO can also give you a paper copy. Avoid edited copies on random sites, as they may be outdated.

Why the source matters

An old or reformatted version may miss a field the RTO now expects, which can mean a second clinic visit.

Print on plain A4 paper at actual size, so the photograph box and signature spaces are not shrunk.

How does Form 1A medical certificate download work on Sarathi?

A Form 1A medical certificate download from Sarathi starts by opening sarathi.parivahan.gov.in and choosing your state. The forms and documents area of the licence services lists the current blank forms, including Form 1A. Exact menu names change, so look for a forms or downloads link.

State sites for Delhi, Gujarat, Maharashtra, Odisha and Assam route licence work through Sarathi, so the same portal usually serves the forms.

Saving a copy

Keep the blank PDF on your phone in case the doctor’s clinic has no printer and you need a second print.

Who can sign medical certificate 1A?

Medical certificate 1A must be signed by a registered medical practitioner, meaning a doctor whose name appears on a state medical council register or the national register kept under the National Medical Commission. The doctor writes their name, registration number, signature, seal and the date.

Government or private doctor

Rule 5 only says registered medical practitioner. A private doctor qualifies, though some transport offices are used to seeing certificates from government hospitals.

Checking the signer

If you are unsure, ask for the registration number and check it against the medical council register. For other kinds of doctor certificates, see our medical fitness certificate guide.

What does a doctor examine on the driving license medical form?

On the driving license medical form, the doctor looks at the abilities a driver needs: eyesight, hearing, use of arms and legs, and any condition that could cause sudden loss of control. The doctor then records an opinion that you are fit or unfit to drive the class applied for.

Checklist of items to carry when a doctor fills the driving license medical form
Bring the glasses you drive with so the eyesight check is accurate.

Bring what you drive with

If you wear glasses, contact lenses or a hearing aid while driving, bring them. The check should reflect how you actually drive.

Tell the doctor about treatment

Mention ongoing treatment or medicines. A doctor who knows the full picture can record conditions properly instead of leaving gaps the RTO will question.

How long is a Form 1A certificate valid?

A Form 1A certificate is valid for one year from the date of issue under Rule 5(2) of the Central Motor Vehicles Rules. If your application is filed or reopened after that year, the RTO can ask for a fresh certificate.

Plan the timing

Get the examination shortly before you apply, not months in advance, so a delayed test date does not push the certificate past its year.

The date that counts

The validity runs from the date the doctor writes, so check that the date is filled in and legible.

How to get a medical certificate for driving license step by step?

To get a medical certificate for driving license use, download Form 1A, fill your part, see a registered doctor in person, check the doctor’s details, and submit it with your application. The whole process usually fits into one clinic visit.

  1. Download the blank form. Get the current Form 1A from Sarathi, your state transport site or the RTO.
  2. Fill your own details. Write your name and address and paste a recent passport-size photograph.
  3. Visit a registered doctor. Go in person with photo ID, your glasses or hearing aid and the printed form.
  4. Check the doctor’s entries. Make sure the form shows the doctor’s name, registration number, signature, seal and date.
  5. Submit with your application. Upload a clear scan with the online application or hand it in at the RTO.

Before you leave the clinic

Read the whole form once. A missing seal is much easier to fix at the clinic than after the RTO rejects the upload.

Keep the original

Keep the signed original safely. Some RTOs ask to see it at the test or verification visit.

How do you upload Form no 1A with an online application?

To upload form no 1A, scan or photograph the signed certificate, save it in the format and size the portal asks for, and attach it at the document upload step of your Sarathi application. Then pay the fee and print the acknowledgement.

Scan quality

Use good light and capture the full page, including the seal and photo. A cropped or blurred scan is the most common reason for an upload query.

File name and order

Name the file clearly, such as “form1a”, and upload it under the medical certificate heading, not under address or age proof.

What is Form 2 driving license?

Form 2 driving license paperwork is the application for a learner’s licence. Rule 10 of the Central Motor Vehicles Rules says it is filed with Form 1A where required, three recent passport-size photographs and the learner licence fee. On Sarathi, Form 2 is the online learner licence application itself.

What goes with Form 2

Form 1 or Form 1A, photographs, proof of age and address, and the Rule 32 fee. Minors applying for a gearless two-wheeler also add guardian consent.

After Form 2

Once you pass the learner test, you get the learner licence in Form 3 and can move to the permanent licence after 30 days.

Form no 2: which form are you looking for?

Form no 2 is a common search, but the number is used by many departments. In licence matters it means the CMVR learner licence application. Other offices use “Form 2” for unrelated documents, so check the department name before you download anything.

How to tell you have the right one

The correct transport Form 2 mentions the Central Motor Vehicles Rules and a learner’s licence near the top.

If you meant something else

If your Form 2 is for a tax, company or land matter, this page cannot help. Use that department’s official website instead.

Form 2 download: where to find the learner licence application?

For a form 2 download, you rarely need a paper copy now. Applying on Sarathi fills Form 2 online as you enter your details. Where an RTO still accepts paper, it provides blank copies at the counter, and state transport sites may list it among forms.

Online is usually faster

The online route checks your entries, books the test slot and takes payment in one session.

Paper copies

If you print a blank form, use the official version and fill it in capitals with blue or black ink.

People searching parents consent form for learning licence Gujarat want the written consent a minor needs. Section 7(2) of the Act lets a person under 18 get a learner licence for a gearless motorcycle only with written consent from the parent or guardian. Gujarat applies this rule through Sarathi.

Who can apply under 18

Section 4(1) sets 16 as the minimum age for a motorcycle up to 50cc. Our learning licence age guide explains the age limits by class.

What we could not confirm

We could not confirm a Gujarat-only printed consent format. Ask your Gujarat RTO or check the Sarathi upload list for Gujarat before you apply.

Why is the medical certificate for license holders of transport vehicles stricter?

The medical certificate for license holders who drive transport vehicles is stricter because these drivers carry passengers or goods for long hours, so a sudden health problem affects many people. That is why Rule 5(1) always asks for Form 1A and why every transport renewal needs it.

Every renewal, any age

A private licence holder under 40 renews with Form 1. A transport licence holder renews with Form 1A, even at 25.

Employers may ask more

Bus operators and fleet companies often run their own checks on top of Form 1A. Those are separate from the RTO requirement.

What do you pay when Form 2 and Form 1A are submitted?

The RTO charges no separate fee for Form 1A. You pay the licence fees under Rule 32: the learner licence fee with Form 2, and test and issue fees with Form 4. Any doctor’s charge for Form 1A is set by the clinic or hospital.

Service under Rule 32Central fee

Driving test, each class₹300
Issue of driving licence₹200

State charges

States may add service, online or smart card charges, so the final amount on Sarathi can be higher.

Doctor’s charge

Government hospitals and private clinics set their own fee for the examination. Ask before you are examined.

Common Form 1A mistakes and how to fix them

Most Form 1A problems are clerical rather than medical. A missing registration number, an old date, no photograph or an unreadable scan causes most RTO queries. Each is easy to fix if you notice it early.

Do not alter it yourself

Never correct dates or details on the form yourself. Ask the doctor to correct and initial any change.

Fake certificates

A certificate issued without an examination is a false document. Using one can lead to the licence being cancelled.

Do you need to submit Form 1A again?

Usually not within the same process. Rule 6 says a person who has already produced a medical certificate for a learner licence, driving licence or added class need not produce it again. The exception is renewal, which needs a fresh certificate where required.

Learner to permanent licence

If you gave Form 1A for a transport learner licence, the RTO generally relies on it at the permanent licence stage, subject to the one-year validity.

At renewal

Plan for a new examination at each renewal from age 40, and at every transport licence renewal.

Which form goes with which licence application?

The fitness form depends on the main application. The learner licence uses Form 2, the driving licence uses Form 4, and renewal uses Form 9. With each, attach Form 1 or Form 1A according to vehicle class and age, as the table below shows.

Medical certificate form 1a questions

What is Form 1A in a driving licence application?

It is the medical certificate a registered medical practitioner signs after examining you. The RTO uses it to judge whether you are physically fit to drive the class you applied for.

Do I need Form 1A for a two-wheeler licence?

Not usually if you are under 40 and the licence is for a private vehicle. A Form 1 self-declaration is enough. At 40 or above, or for a transport class, Form 1A is needed.

Can any doctor sign Form 1A?

It must be a registered medical practitioner, that is a doctor registered with a state medical council or the national register. Ask the doctor to write the registration number clearly.

How long is a Form 1A medical certificate valid?

One year from the date of issue under Rule 5(2) of the Central Motor Vehicles Rules. Use it within that year or get a fresh examination.

Is Form 1A free?

The RTO does not charge a separate fee for it. A government hospital or private doctor may charge for the consultation and examination, and that amount varies.

Is Form 1 the same as Form 1A?

No. Form 1 is your own declaration of physical fitness. Form 1A is a certificate from a doctor. They share a number but are different documents.

Can I get Form 1A online without seeing a doctor?

No genuine route does that. The blank form can be downloaded, but the doctor must examine you in person before signing. A certificate bought without an examination is a false document.

What is Form 2 for a driving licence?

Form 2 is the application for a learner's licence under Rule 10. It is filed with Form 1 or Form 1A, photographs and the learner licence fee.

Does a minor need parental consent for a learner licence in Gujarat?

Yes, for the only licence a person under 18 can get, a gearless motorcycle learner licence. Section 7(2) requires written consent from the parent or guardian. Check the current format with the Gujarat RTO.
