Medical fitness certificate: who issues it, what it shows and how to get one
A medical fitness certificate is a signed statement from a registered medical practitioner that you were examined on a given date and found fit for a stated purpose, such as a job, a course or a driving licence. Get the format from whoever asked for it, visit a government hospital or registered doctor, complete the checks, and make sure the doctor adds a registration number, signature and seal.
Key takeaways
- The body that asks for a medical fitness certificate sets its format; there is no single national form for jobs or students.
- Only a registered medical practitioner should sign it, with the medical council registration number written clearly.
- For a driving licence, the Motor Vehicles Act requires a doctor's Form 1A for transport licences and for renewals at age 40 and above.
- Government jobs often name a government hospital or medical board, so check the joining letter before you book.
- Fees and validity are set by each hospital and requester; nobody should sell a certificate without an examination.

What is a medical fitness certificate?
A medical fitness certificate is a dated document in which a registered doctor confirms that they examined you and found you fit for a named purpose. Employers, colleges, sports bodies and the RTO ask for it. It is an opinion based on the examination, not a lifelong guarantee of health.

Why organisations ask for it
An employer wants to know a new recruit can safely do the job. A college wants to know a student can take part in classes, labs and sports. A licensing authority wants to know a driver can see, hear and react well enough to drive.
What it is not
It is not a medical report listing every test result, and it is not an insurance document. It also does not have to reveal a diagnosis. The doctor states the findings the form asks for and a clear fit or unfit opinion.
Medical certificate and fitness certificate: are they the same thing?
People often say medical certificate and fitness certificate as if they mean one paper, but they cover two needs. A medical certificate is the broad term for any doctor’s statement. A fitness certificate is one kind of it, stating that you are fit for work, study or an activity.
The broader medical certificate
A sick note for leave, a disability certificate, an age-estimation certificate and a cause-of-illness letter are all medical certificates. Each answers a different question and follows its own format.
The narrower fitness certificate
A fitness certificate looks forward: can this person start a job, join a course or drive safely from now on? It ends with words such as “fit for duty” or “fit to drive”.
Who can issue a medical fitness certificate?
A registered medical practitioner can issue a medical fitness certificate. That means a doctor whose name is on a state medical council register or the national register kept under the National Medical Commission, set up by the NMC Act, 2019. Some posts require a government doctor or a medical board instead.
Government doctors and medical boards
Government departments, police and defence recruitment, and some public sector units ask for a certificate from a civil surgeon, a district hospital or a board of doctors. The joining letter usually names the place.
Private registered doctors
For most private jobs, colleges and sports events, a certificate from any registered MBBS or higher doctor is accepted. If the form says “registered medical practitioner”, check that the doctor writes the registration number.
What does a doctor check before signing?
The doctor checks the items the form lists, usually height, weight, blood pressure, pulse, vision and hearing, plus a general look at the heart, lungs, limbs and joints. They also ask about past illness, surgery and regular medicines, because those answers shape the fitness opinion.
The basic physical exam
A basic exam is usually done in one sitting at the clinic. The doctor listens to the chest, checks reflexes and posture, and asks you to read a vision chart. Colour vision is checked with a test book when the job or licence needs it.
Your own history
You will be asked about conditions such as epilepsy, diabetes, heart disease or mental health treatment. Answer honestly: a hidden condition discovered later can cancel the certificate and the job or licence based on it.
Which details must every certificate carry?
Every genuine certificate should show your name, age, sex and an identification mark, an attested photograph, the date and place of the examination, the findings, and the doctor’s opinion. It must end with the doctor’s name, qualification, registration number, signature and seal. Missing any of these invites rejection.

Your identity details
The name should match your Aadhaar, PAN or school records exactly. Many forms also ask for a visible mark of identification, such as a mole or scar, so the certificate cannot be swapped to another person.
The doctor’s details
The registration number with the council’s name is what lets anyone verify the signer. A rubber seal with the hospital or clinic name and a legible signature complete the authentication.
Simple medical certificate format: what fits on one page?
A simple medical certificate format fits on one page: a heading, your details with photo, a short statement that you were examined, a fit or unfit line, and the doctor’s block at the bottom. It is used where the requester has not supplied its own form.
A typical one-page layout
The heading reads “Medical Certificate of Fitness”. Below it come your name, parent’s name, age, address and a photo. The body says the doctor examined you on a date and found no disease or disability that would stop you from the stated purpose.
When a simple format is enough
Coaching classes, gyms, short courses, some private employers and hostel admissions often accept a one-page certificate. For anything involving safety, heavy work or government service, expect a longer form with test results.
How should a medical certificate letter format be written?
A medical certificate letter format is written on the doctor’s or hospital’s letterhead in plain sentences rather than as a filled form. It names you, states the date of examination, gives the fitness opinion, mentions any limits, and ends with the signature, registration number and seal.
Wording that works
Clear wording reads like: “This is to certify that I have examined [name], aged [age], on [date], and found them medically fit to [purpose].” Doctors avoid jargon so that an HR officer or principal can read it.
Adding conditions
If you are fit with conditions, the letter says so: “fit for desk work, to avoid lifting heavy loads for three months”, or “fit to drive with corrective glasses”. Conditions protect both you and the doctor.
Medical physical fitness certificate format: which tests are listed?
A medical physical fitness certificate format lists measured items: height, weight, chest expansion, blood pressure, pulse, vision for distance and near, colour vision, hearing and a check of limbs and spine. Some formats add blood group, haemoglobin, urine tests and a chest X-ray.

Measurements in boxes
These formats have boxes for each reading so that a recruiter can compare candidates against the post’s standard. Police, security and field jobs often set minimum height, chest or vision standards in their notification.
Laboratory tests
Tests are done only if the form asks. Food, hospital and childcare jobs may ask for screening tests. Keep the lab reports; some employers want them attached to the certificate.
What is a medical certificate of fitness form?
A medical certificate of fitness form is a pre-printed form supplied by the requester, with fixed questions for you and fixed boxes for the doctor. Unlike a letter, nothing can be left out, which is why government departments and large companies prefer it.
Part filled by you
The first part is a declaration of your medical history. You sign it, sometimes in front of the doctor, confirming the answers are true. False answers can lead to dismissal later.
Part filled by the doctor
The second part records findings and the opinion. The doctor signs each page, stamps it and attests your photo. A form with pages unsigned is often returned.
Doctor fitness certificate format: what the doctor fills in
In any doctor fitness certificate format, the doctor’s section records findings, a final opinion and their identity. The opinion is usually “fit”, “unfit” or “temporarily unfit”, sometimes with a review date. The doctor’s block includes name, degree, registration number with council, signature and seal.

Temporarily unfit
“Temporarily unfit” means a fixable problem, such as uncontrolled blood pressure or a healing fracture. The doctor may ask you to come back after treatment for a fresh opinion.
Why the registration number matters
The council registration number is the doctor’s licence to practise. Without it, a certificate cannot be traced, and many HR teams and RTOs reject it outright.
Medical fitness certificate for job pdf: what do employers ask for?
When people search for a medical fitness certificate for job pdf, they usually want a printable form to take to a doctor before joining. Employers normally send their own form with the offer letter. Use that one; a random PDF may not have the boxes they need.
What the offer letter usually says
It names the format, the type of doctor or hospital, and the date by which the certificate must reach HR. Some companies use a tie-up clinic and pay for the exam directly.
Government jobs
For a government post, the appointment letter often specifies a civil surgeon, district hospital or medical board. A private doctor’s certificate is not a substitute in those cases, however detailed it is.
Is a medical fitness certificate sample pdf safe to use?
A medical fitness certificate sample pdf from the internet is safe as a guide to what the doctor will write, but submit only what the doctor actually fills and signs. Never fill a sample with your own details and then look for someone to stamp it without an exam.
Good uses of a sample
A sample helps you see the questions in advance, gather your medical history and carry old prescriptions. It also helps a doctor who has never seen the requester’s format.
Where samples go wrong
Some downloadable samples include outdated wording, wrong council names or fields the requester does not need. Always prefer the employer’s or institution’s form, even if a sample looks more polished.
How to get a medical fitness certificate from a government hospital?
To get a medical fitness certificate government hospital doctors sign, go to the outpatient department with the format, photo ID and photographs, register for the general medicine OPD, complete the examination and any tests, and collect the signed certificate. Some hospitals have a separate counter for certificates.
- Get the format. Ask the employer, college or RTO for its own form, or check the joining letter or prospectus.
- Register at the OPD. Go to a government hospital or registered doctor with photo ID, passport photos and the form.
- Complete the examination. The doctor checks height, weight, blood pressure, vision and other items the form lists.
- Do any tests asked for. Give blood or urine samples or have an X-ray only if the form requires it.
- Collect the signed certificate. Check that it carries the doctor’s name, registration number, signature, seal and date.
Timing your visit
Government OPDs register patients in the morning, and certificate desks may work on fixed days. Arrive early, and if tests are needed, expect to return the next day with reports.
Medical boards
Some posts need a board of two or three doctors. The department fixes the date and sends you a letter. Bring the letter, ID and any old reports, and expect a longer examination.
Government hospital or private clinic: which should you choose?
Choose a government hospital when the requester names one or when the job is a government post. Choose a private registered doctor when the requester only says “registered medical practitioner” and you need the certificate quickly. Both are valid if the doctor is registered and the format is correct.

| Point | Government hospital | Private registered doctor |
|---|---|---|
| Accepted for government posts | Usually required | Only if the order allows it |
| Speed | Queue, fixed OPD hours | Often same day |
| Charges | Nominal OPD or certificate fee | Set by the clinic |
| Tests | In-house lab, may take a day | Outside lab, extra cost |
| Best for | Government jobs, medical boards | Private jobs, colleges, sports |
When the requester is silent
If the form simply asks for a registered doctor, a private clinic is fine. Keep the doctor’s phone number in case HR wants to confirm the certificate.
Avoid “certificate only” counters
Shops or agents offering a certificate without examination are not clinics. A certificate issued that way can be cancelled, and using it can lead to legal trouble.
How much does a medical fitness certificate cost?
There is no fixed national fee for a medical fitness certificate. Government hospitals usually charge a small OPD or certificate fee set by the state or hospital, and private clinics set their own consultation charge. Any blood tests, urine tests or X-rays the form needs are billed separately.
What changes the cost
A one-page certificate after a short exam costs least. Long formats with lab tests and an X-ray, a medical board or a specialist opinion cost more. Ask the hospital counter before you register.
Who pays
For many private jobs, the employer pays through a tie-up clinic. For students and job seekers, you usually pay. The licence application fee on Sarathi does not include the doctor’s charges for Form 1A.
How long is a medical fitness certificate valid?
A medical fitness certificate has no fixed national validity. The employer, college or authority decides how recent it must be, and many state that in the form or letter. If no period is stated, ask before you are examined, because a certificate that is too old will simply be refused.
Why validity is short
Health changes. Blood pressure, vision and some conditions can shift within months, so requesters want a recent exam. A certificate dated long before joining often has to be redone.
Licence certificates
For a driving licence, use a Form 1A signed close to the application date. Your state transport office can tell you how recent it must be when you upload it.
What is a health fitness certificate?
A health fitness certificate is another name for a medical fitness certificate, used mostly by employers, gyms and hostels. It confirms you are in good general health and free from conditions that would put you or others at risk in the stated activity.

Common users
Hostels, residential schools, gyms, swimming pools, adventure camps and some apartment-based services ask for a health certificate before allowing you in.
Health versus fitness wording
“Health certificate” tends to stress freedom from infection, while “fitness certificate” stresses physical ability. Many forms combine both and ask the doctor to confirm each.
Physical fitness and health certificate: when are both asked for?
A physical fitness and health certificate is asked for when the activity is physically demanding and also involves close contact with others: sports academies, defence coaching, police training, mountaineering and some field jobs. The form covers strength and stamina as well as general health and any infection risk.
Typical extra checks
Expect a check of flat feet, knock knees, spine and joints, and sometimes a short exercise test. Some formats ask the doctor to confirm you can run, swim or lift a stated weight.
Preparing for it
Sleep well, avoid heavy exercise the day before, and bring glasses or lenses if you wear them. If you take regular medicine, carry the prescription.
What is a general fitness certificate?
A general fitness certificate states that you are fit in general terms, without tying the opinion to one job or test. It is used for short courses, travel groups, volunteering and private training where the organiser only needs basic assurance.
Limits of a general certificate
Because it is broad, it may not satisfy a requester with specific standards. A security agency or factory job will usually ask for a detailed form instead.
Asking for the right wording
Tell the doctor what the certificate is for, even if the form is general. A purpose line such as “fit to attend a residential course” helps it be accepted.
Fitness certificate for students: when do schools and colleges ask?
A fitness certificate for students is asked for at admission to many colleges, hostels, medical and nursing courses, sports quotas, NCC and residential schools. The institution usually prints a format in its prospectus or sends it with the admission letter.
What the form checks
Student forms usually record height, weight, vision, hearing, vaccinations and any conditions such as asthma or allergies that staff should know about. They protect the student as much as the institution.
Minors
For students under 18, a parent or guardian signs the history part. Bring the child’s vaccination card and earlier medical records to the doctor.
What does medical feet certificate mean?
“Medical feet certificate” is a misspelling of “medical fit certificate”, meaning a medical fitness certificate. There is no separate certificate about feet. If your form or recruiter asks about feet specifically, it is usually checking for flat feet as part of a physical fitness exam.
Flat-foot checks
Police, defence and some security jobs check for flat feet because they affect long marches and standing duty. This is part of the physical exam, not a separate document.
What to ask for
Ask the doctor for a medical fitness certificate on the format your recruiter gave you. If they need a foot check, the format will have a line for it.
When does a driving licence need a medical fitness certificate?
A driving licence needs a doctor’s medical fitness certificate, Form 1A, for a transport vehicle licence and for renewal once you turn 40, under sections 8(3) and 15(1) of the Motor Vehicles Act. The RTO may also ask for one at the driving test. Detailed form-filling help is on our medical certificate Form 1A page.

| Licence situation | Doctor’s certificate needed? | Legal basis |
|---|---|---|
| Learner licence, private car or two-wheeler | No, self-declaration in Form 1 | Section 8(3) proviso |
| Learner licence, transport vehicle | Yes, Form 1A | Section 8(3) |
| Driving licence test | Only if the licensing authority asks | Section 9(3) |
| Renewal at age 40 or above | Yes, Form 1A | Section 15(1) |
| Renewal of a transport licence | Yes, Form 1A | Section 15(1) |
Upload or carry it
On Sarathi you upload the signed certificate as a scanned document where it applies, and some offices ask to see the original. Keep a clear scan.
Same doctor rules apply
The licence form must be signed by a registered medical practitioner, just like a job certificate. A clinic stamp alone is not enough.
Self-declaration or doctor’s certificate: which applies to your licence?
If you are under 40 and applying for a private-vehicle licence, a self-declaration in Form 1 is usually enough. If you are 40 or older, or want a transport licence, you need a doctor’s Form 1A. The difference depends on age and vehicle class, not on the state.
What Form 1 covers
Form 1 asks you to declare conditions such as epilepsy, blackouts, poor vision or loss of a limb. A false declaration can lead to the licence being cancelled.
When a doctor is needed anyway
If you declare a condition in Form 1, the licensing authority can still ask for a doctor’s opinion before it issues the licence.
Does a learner licence application need a doctor’s certificate?
A learner licence for a private car or two-wheeler does not need a doctor’s certificate under the proviso to section 8(3); a self-declaration is used instead. A learner licence for a transport vehicle does need a signed Form 1A. The rest of the process is on our learner licence test questions guide.
Age 40 and above
Transport departments ask applicants aged 40 or older to attach a doctor-signed Form 1A even for a private vehicle, so check your age before choosing between Form 1 and Form 1A.
Before the online test
Have the scan ready before you start the Sarathi form, so the upload step does not time out.
Is a medical check needed to renew a licence after 40?
Yes. Section 15(1) of the Motor Vehicles Act says a renewal application from a holder who has reached 40 must include a medical certificate in the prescribed form. That form is Form 1A. Steps, fees and late renewal rules are covered on our DL renewal page.
Why 40
Vision and reaction time start to change with age, so the law adds a doctor’s check at the first renewal after 40 and keeps it for later renewals.
Get it before applying
Book the doctor first, then start the online renewal. A Form 1A signed after you have paid may delay the application.
Why do commercial drivers face a stricter medical check?
Drivers of buses, trucks, taxis and other transport vehicles carry passengers or heavy loads, so the law requires a doctor-signed Form 1A at first issue and every renewal, whatever their age. The doctor pays special attention to vision, hearing, blood pressure and conditions that cause sudden loss of control.
What employers add
Transport companies and fleet operators often run their own annual checks on top of the licence certificate. Those are company policies and use the company’s form.
Keep a copy in the cab
Some operators ask drivers to keep the latest certificate with their licence papers. A scan on the phone is useful during inspections.
Medical certificate vs vehicle certificate: what is the difference?
A driver’s medical fitness certificate is about the person: it says you can drive safely. A vehicle fitness certificate is about the vehicle: it says a transport vehicle passed its roadworthiness test. They are separate documents from separate processes; see our guide to the vehicle fitness certificate for the vehicle side.
Which one a driver needs
A taxi or truck driver often needs both: their own Form 1A for the licence, and a valid fitness certificate for the vehicle they drive. One cannot replace the other.
Where each is checked
Your medical certificate is checked by the licensing authority. The vehicle certificate is checked at the RTO’s inspection and appears in the Vahan record.
Medical fitness certificate West Bengal: where do residents get one?
Residents looking for a medical fitness certificate West Bengal offices accept can go to the OPD of a government district or sub-divisional hospital, a medical college hospital, or a registered private doctor. The format comes from the employer, institution or, for a licence, the central Form 1A used on Sarathi.
For state government jobs
West Bengal government recruitment letters usually name the hospital or medical board for the examination. Follow that letter rather than a general online format.
For a licence in West Bengal
West Bengal licence services run on Sarathi like other states, so the same Form 1A and age rules apply. Check the driving licence hub for the overall process.
Health certificate Rajasthan: what are people looking for?
Searches for “health certificate Rajasthan” mix several needs: a fitness certificate for a Rajasthan government job, a student certificate for a state college, or Form 1A for a licence. We could not confirm any single Rajasthan-specific health certificate format, so use the format your requester gives you.
Government hospitals in Rajasthan
District hospitals and medical college hospitals in Rajasthan issue fitness certificates through their OPDs, like hospitals in other states. Ask at the hospital which counter handles certificates.
Not to be confused
A health insurance card or a health scheme card is not a fitness certificate. If a form asks for a “health certificate”, it means a doctor’s examination and signature.
Can you get a medical fitness certificate online?
You can download a blank format online, and some hospitals let you book the OPD slot online, but the examination itself has to be in person. Vision, blood pressure, hearing and physical checks cannot be done over a video call, so most requesters reject purely online certificates.
Where telemedicine helps
An online consultation is useful for follow-up questions, prescriptions or deciding which tests you need. It is not a substitute for the exam a fitness form describes.
Websites selling instant certificates
Sites promising a certificate in minutes without a doctor seeing you are a red flag. The certificate may be traced back and rejected.
Can a doctor refuse to certify you as fit?
Yes. A doctor must certify only what they find, so they can decline to sign “fit” if they find a condition that affects the purpose. They may instead write “temporarily unfit”, ask for tests, or refer you to a specialist before giving an opinion.
What you can do
Ask what was found and whether treatment, glasses or medicine can fix it. Many unfit results are reversed after a short treatment period.
Getting a second opinion
You can see another registered doctor or ask the requester for a medical board. Tell the second doctor about the first result; hiding it can make both certificates questionable.
What if the certificate has a mistake?
If the certificate has a wrong name, age, date or missing field, go back to the same doctor. Only the signing doctor should correct it, and they should initial and stamp the change. Never correct it yourself, even for a small spelling error.
Small errors
A spelling mismatch with your ID is the most common problem. The doctor can correct it with an initial or issue a fresh certificate.
Big errors
If the purpose, findings or date are wrong, ask for a new certificate. A heavily corrected form looks tampered with and may be refused.
Why is a fake medical certificate a serious risk?
A fake or bought medical certificate can lead to cancellation of the job, admission or licence, and making or using a false certificate can be a criminal offence. For drivers it also puts lives at risk, because a hidden condition affects everyone on the road.
How fakes are spotted
HR teams and RTOs check registration numbers, call hospitals and compare signatures. A number that does not match the doctor’s name is a quick giveaway.
Doctors face action too
A doctor who signs without examining a patient can face disciplinary action from the medical council, so genuine doctors refuse such requests.
What should you carry to the medical examination?
Carry the requester’s form, photo ID, two or three passport photos, your glasses or contact lenses, old prescriptions, recent test reports and any letter from your employer or college. Having these ready usually means one visit instead of two.
Photographs
Most forms need a photo pasted on the certificate, which the doctor signs across. Bring extra copies for hospital records.
Medical records
If you have a long-term condition, bring the latest report from your treating doctor. It helps the examining doctor give a fair opinion.
How to check a doctor’s registration number?
Read the registration number and council name on the certificate, then search for the doctor on the state medical council’s register or the national register under the National Medical Commission. The name, qualification and registration year should match what is on the certificate.
When the search shows nothing
Some older or state-only registrations take time to appear online. Call the state council or ask the doctor for a copy of their registration certificate.
Why recruiters check
Checking takes minutes and stops most fake certificates. Expect a careful employer or RTO to do it.
Do food handlers and factory workers need a health certificate?
Food businesses and factories often need staff medical examinations under their own food-safety or workplace rules, and these are separate from a general job certificate. The rules depend on the industry and the state, so ask the employer which format and doctor they need.
Food businesses
Restaurants, caterers and food factories may need workers’ medical checks as part of their food-licence conditions. This is a food-safety matter, not a transport service; the food safety officer for your area can confirm what applies.
Factories and heavy work
Hazardous or physically demanding work may require checks by a doctor appointed for that purpose. Your employer’s safety officer will tell you the schedule.
Return to work after illness: what certificate is needed?
After a long illness, surgery or medical leave, many employers ask for a fitness-to-resume certificate from the treating doctor. It states that you have recovered enough to return, with any limits such as lighter duties for a few weeks.
Sick note versus fitness note
A sick note covers the time you were away. A fitness note covers your return. Employers usually want both, and government offices have their own forms for each.
Phased return
If you are fit only for part-time or lighter work at first, ask the doctor to say so. Clear limits help HR arrange suitable duties.
Common certificate mistakes and how to fix them
Most rejected certificates fail on paperwork, not health: no registration number, the wrong format, an old date, a name mismatch or an unattested photo. Each is easy to fix if you check the certificate before leaving the clinic.

| Mistake | Why it is rejected | Fix |
|---|---|---|
| No registration number | Signer cannot be verified | Doctor adds it with an initial |
| Wrong format | Requester’s boxes missing | Doctor fills the requester’s form |
| Old date | Health may have changed | Fresh examination |
| Name differs from ID | Identity doubt | Doctor corrects and initials |
| Photo not attested | Could be swapped | Doctor signs across the photo |
Check before you leave
Read every line at the counter. It takes two minutes and avoids a second trip to a busy OPD.
Keep copies
Scan the signed certificate and keep the original flat in a folder. Many employers and the RTO ask for both.
Which certificate type matches which purpose?
Match the certificate to its purpose: a detailed form for jobs and training, a one-page letter for short courses, Form 1A for licences, and a fitness-to-resume note after illness. Using the wrong type is the second most common reason for rejection after a missing registration number.
| Purpose | Usual type | Signed by | Format source |
|---|---|---|---|
| Government job | Detailed form or medical board | Government doctor or board | Appointment letter |
| Private job | Detailed form or letter | Registered doctor | Offer letter |
| College or hostel | Student form | Registered doctor | Prospectus |
| Driving licence | Form 1A | Registered medical practitioner | Central form on Sarathi |
| Return after illness | Fitness-to-resume note | Treating doctor | Employer |
If two requesters overlap
If both your college and a sports body want certificates, ask whether one can accept the other’s form. Often each insists on its own.
How do you prepare for the day of the exam?
Prepare by sleeping well, eating normally and avoiding tea, coffee or smoking just before a blood pressure check. Fast only if the form includes a fasting blood test. Wear clothes that let the doctor check your arms, chest and legs easily.
After the exam
Ask when the certificate will be ready, especially if tests are pending. Note the counter or room where you collect it.

Can one medical fitness certificate be reused?
Usually no. Each requester wants its own format with a recent date and a purpose line that names it. A certificate made for one job is rarely accepted for another, and a driving licence needs its own Form 1A regardless of other certificates you hold.
When reuse can work
A small private course might accept a copy of a recent job certificate. Ask first and offer to show the original.
Licence checks
You can check your licence details at any time with our online licence check guide; the medical certificate itself is not shown there.

Who can issue a medical fitness certificate in India?
A registered medical practitioner, that is a doctor registered with a state medical council or the National Medical Commission. Some government posts insist on a government hospital doctor or a medical board.
Is there a standard medical fitness certificate format?
No single national format exists for jobs or students. Each employer or institution supplies its own. Driving licences are the exception: they use the central Form 1A.
How much does a medical fitness certificate cost?
There is no fixed national fee. Government hospitals usually charge a nominal OPD or certificate fee, while private clinics set their own charges. Tests such as blood work or an X-ray cost extra.
How long is a medical fitness certificate valid?
Validity is decided by the body that asks for it. Many want a certificate issued shortly before joining or admission. If no period is stated, ask them before you get examined.
Can I get a medical fitness certificate online?
An online consultation cannot measure vision, blood pressure or hearing, so most employers and RTOs will not accept it. You can download a blank format online, but the exam must be in person.
Do I need a medical certificate for a learner licence?
For a private car or two-wheeler, applicants under 40 usually give a self-declaration in Form 1. A transport vehicle licence needs a doctor-signed Form 1A.
Is a medical certificate needed to renew my driving licence?
Yes, if you are 40 or older, or if you hold a transport licence. Section 15(1) of the Motor Vehicles Act requires a medical certificate with those renewals.
What is a medical feet certificate?
It is a common misspelling of medical fit certificate. There is no separate document for feet; people searching this want a standard fitness certificate.
Can a homeopathic or ayurvedic doctor sign a fitness certificate?
It depends on the requester. Many employers and the RTO ask for a doctor registered under the medical council system. Check the form's wording before you choose a doctor.
What if my certificate says unfit?
Ask the doctor what condition was found and whether treatment, glasses or a specialist opinion could change the result. You can be re-examined later. Do not try to get a second certificate hidden from the first.
Can I use one certificate for two purposes?
Only if both requesters accept the same format and date. Usually a job certificate and a driving licence Form 1A are separate documents.
Is a fitness certificate the same as a sick leave certificate?
No. A sick leave note says you were unwell for a period. A fitness certificate says you are fit to start or resume work or another activity.
