Heavy vehicle license and two-wheeler licence applications without name errors
A heavy vehicle license is an extra class added to an existing driving licence after you clear a learner stage, a Form 1A medical and a driving test on Sarathi. Motorcycle and scooter licences follow the same Form 2 to Form 4 route. Type your name exactly as on Aadhaar before paying, because a wrong spelling later needs a separate correction.
Key takeaways
- A heavy vehicle license is added to the same DL number; you apply for a learner licence in that class first.
- The licence application form is Form 2 for the learner stage and Form 4 for the driving licence; Sarathi generates both.
- India has no provisional license; the matching stage is the learner's licence printed in Form 3.
- Two-wheeler classes are MCWG (with gear) and MCWOG (without gear); a 50cc scooter is allowed from 16.
- West Bengal, Himachal Pradesh, Maharashtra and Gujarat (Surat, GJ-05) all use the national Sarathi screens.
- A name typed wrongly on the application stays on the card until you apply for a correction.

What is a heavy vehicle license?
A heavy vehicle license is a driving licence class that lets you drive large goods carriers and buses, printed on the card with codes such as HTV, HGMV, HPMV or TRANS. It is not a separate document: the class is added to your existing licence number after a learner stage and a driving test.

Classes you may see on the card
Cards from different states print the heavy class differently. HTV means heavy transport vehicle, HPMV heavy passenger motor vehicle and TRANS covers heavy goods and passenger vehicles plus trailers. For every code and its full form, read our HMV licence guide.
Why the class sits on the same licence
A licence in India is one record per person on Sarathi. When you pass the heavy test, the RTO adds the new class and its validity to that record and prints a fresh Form 7 card. Your name, date of birth and licence number carry over unchanged.
Who can apply for a heavy vehicle license?
You can apply if you are at least 20 years old (the age set for transport vehicles in section 4(2) of the Motor Vehicles Act) and already hold a driving licence for a light motor vehicle. Section 7(1) of the Act still asks for that LMV licence to be held for one year before a transport learner’s licence.
Medical fitness
Every transport class application needs a Form 1A medical certificate signed by a registered doctor. Applicants aged 40 or above need it for any class. Carry the original to the test, because some RTOs check the doctor’s seal against the photo.
Training and endorsements
Rule 14 of the CMVR lets you attach a Form 5 certificate from a driving school, and some states ask for one before a heavy test. Separate endorsements exist for hill driving and hazardous goods. Your state’s Sarathi checklist shows which ones it asks for.
Heavy vehicle license applications: get your name right first time
The most common reason a heavy vehicle license card comes back wrong is a name typed differently from the proof. Sarathi copies your existing LMV record into the new application, so check that the name, father’s name and date of birth there already match your Aadhaar before you add a class.
Check the old record first
Open the licence details on Sarathi and compare them with your Aadhaar and school certificate. If the old card uses initials, a shortened surname or an old married name, fix that record before you start the heavy class. Otherwise the error is printed again on the new card.
What a mismatch costs you
A wrong name blocks Aadhaar e-KYC, can stop insurance claims after an accident and can make a transport employer reject your papers. Fixing it later is a separate service with its own fee, proof and wait. For that process, see our guide on how to change name in driving license.
What is a licence application form?
A licence application form is the statutory form you submit to the licensing authority under the Central Motor Vehicles Rules, 1989. The learner’s licence uses Form 2 and the driving licence uses Form 4. Today both are generated by Sarathi from the details you type online.

Paper form or online entry?
Most states have moved fully online. You fill the fields on Sarathi, and the portal prints a completed form with your photo and signature. A few RTO counters still hand out paper forms for walk-in applicants, but the details are typed into Sarathi anyway.
What the form asks for
Expect your full name, the name of your father, mother or spouse, date and place of birth, address, vehicle class and contact details. Every field is printed on the card or kept in the record, so read the preview screen slowly before you pay.
Which license application form fits each stage?
Each stage of a licence has its own license application form: Form 1 or 1A for fitness, Form 2 for the learner’s licence, Form 4 for the driving licence, and further forms for renewal and other changes. The table below matches each form to its use under the CMVR.
| Form | What it is | When you use it |
|---|---|---|
| Form 1 | Self-declaration of physical fitness | With most learner and licence applications |
| Form 1A | Medical certificate from a registered doctor | Transport classes and older applicants |
| Form 2 | Application for a learner’s licence | Every new class, including heavy |
| Form 3 | The learner’s licence itself | Printed after you pass the learner test |
| Form 4 | Application for a driving licence | After the learner waiting period |
| Form 5 | Driving school certificate | Where the state or class needs training proof |
| Form 7 | The driving licence card | Issued after the test is passed |
Forms for later changes
Renewal, a duplicate licence and a change of address or name use other forms that Sarathi lists under its licence services. You choose the service online, and the portal attaches the right form to the application.
Keeping the printouts
Save the acknowledgement and the completed form as PDFs. The RTO may ask for them at the test, and they hold the application number you need for the status check.
What is a provisional license?
A provisional license is the first licence a learner driver gets in the United Kingdom and a few other countries, before passing the practical test. India does not use the term. The Indian stage that does the same job is the learner’s licence, issued in Form 3.

Why Indians search for it
The word appears in foreign driving guides, video tutorials and visa paperwork, so many people look for a provisional licence in India. On Sarathi, look for Apply for Learner Licence instead.
Foreign provisional licences in India
A provisional licence from the UK or elsewhere does not let you drive in India. Visitors need a full licence and, usually, an International Driving Permit.
Is a provisional license the same as a learner’s licence?
In purpose, yes: both let a new driver practise under supervision before a test. The rules differ. An Indian learner’s licence is valid for six months, needs an instructor with a valid licence beside you, and requires an L sign on the front and rear of the vehicle.
Learner licence rules in short
You may drive only with a licensed person who can control the vehicle. On a two-wheeler you may not carry another passenger except as the rules allow. The licence covers only the classes printed on it.
Moving to a full licence
You can book the driving test 30 days after the learner’s licence is issued and before it lapses. If it expires, you apply for a new learner’s licence and pay again.
What is a motorcycle license?
A motorcycle license is a driving licence that includes a two-wheeler class. Section 10(2) of the Motor Vehicles Act lists two such classes: motor cycle without gear (MCWOG) and motor cycle with gear (MCWG). Both are added to the same licence record as any car or heavy class you hold.
How it appears on the card
The class column shows MCWG or MCWOG, sometimes written as M/CYCL.WG or MC Without Gear on older cards. Each class has its own date of issue. To decode every abbreviation, read our MCWG full form explainer.
Riding without the right class
Section 3 of the Act says nobody may drive in a public place without a licence that covers the vehicle. Riding a geared bike on a gearless-only licence can be treated as driving without a valid licence for that vehicle.
Motorcycle license with gear or without gear: which class?
Choose MCWG if you will ride any bike with a clutch and gear lever, and MCWOG if you will ride only scooters, mopeds or electric scooters. Many first-time riders apply for both classes on one learner application, which needs one learner test and a separate driving test for each class.

What insurers and RTOs accept
Insurer guides say an MCWG licence is accepted for gearless scooters as well. The Act, however, treats the classes separately, and claim teams read the class on your card. If you ride both types, holding both classes removes any doubt.
Bring the right vehicle to the test
The test is taken on the type of vehicle you applied for. Bring a geared bike for MCWG and a scooter for MCWOG, with valid RC, insurance and PUC papers.
What is a scooter licence?
A scooter licence is a licence with the MCWOG class, covering two-wheelers without a gearbox such as scooters, mopeds and most electric scooters. You get it through the same Sarathi route as any other class: learner’s licence first, then a driving test on a gearless vehicle.
Electric scooters and licences
Low-speed electric scooters that are exempt from registration may not need a licence, but most electric scooters sold today are registered vehicles. If your scooter has a number plate, you need a valid licence to ride it.
Scooty and similar names
Riders often say “scooty licence” after a popular model. Legally it is the same motor cycle without gear class, so search for that on Sarathi.
Can you get a scooter licence at 16?
Yes, but only for a two-wheeler with an engine of 50cc or less. Section 4(1) of the Act sets 16 as the age for such motorcycles, and section 7(2) asks for the written consent of a parent or guardian. Every other scooter or bike needs you to be 18.
What the guardian signs
The consent is part of the learner application. The guardian’s name must match their own ID proof, and your name must match your school or birth certificate. A mismatch here is a common reason for a rejected learner application.
At 18
When you turn 18 you apply for any other class you need. Your licence number stays the same, and the new class is added to the card.
How does a 2 wheeler license apply work online?
A 2 wheeler license apply process starts on Sarathi: choose your state, open the learner licence service, select MCWG, MCWOG or both, fill the form, upload proof, pay, and pass the learner test. After the waiting period you apply for the driving licence and book a test at the RTO.
- Choose your state on Sarathi. Open sarathi.parivahan.gov.in and pick the state where you live.
- Apply for a learner licence. Fill the Form 2 entry, pick the vehicle class and type your name exactly as on Aadhaar.
- Upload and pay. Upload proof, photo and signature, then pay the fee shown on screen.
- Pass the learner test. Clear the learner test and download the Form 3 learner licence.
- Apply for the driving licence. After the waiting period, fill the Form 4 entry and book a driving test slot.
Aadhaar and contactless services
Since 3 March 2021, MoRTH has allowed several licence services to be completed online with voluntary Aadhaar authentication. Where your state offers it, the learner test can be taken from home. Aadhaar authentication works only when your typed name matches the Aadhaar record.
When to visit the RTO
The driving test always happens in person, on a track or road chosen by the RTO. Carry the original documents you uploaded, the learner licence and a roadworthy vehicle of the right class.

What do these licence classes cost?
The central fee for each stage is set by CMVR Rule 32: Rs 150 per class for a learner’s licence, Rs 50 for the learner test, Rs 300 per class for the driving test and Rs 200 to issue the licence. A smart card adds Rs 200. States add service, card and postal charges on top.
| Stage | Central fee (Rule 32) |
|---|---|
| Learner’s licence, each class | ₹150 |
| Learner’s licence test or retest | ₹50 |
| Driving test, each class | ₹300 |
| Issue of driving licence | ₹200 |
| Smart card licence | ₹200 extra |
What your receipt will show
Your Sarathi payment screen lists the state’s full amount before you pay, including any service charge. That screen is the figure to trust. For worked totals by class, including heavy classes, see our heavy licence fees breakdown.
Adding a heavy class
Adding one heavy class to an existing licence uses the learner and test rows above for that class, plus any addition-of-class or card charge your state shows. There is no separate central fee called a heavy vehicle fee.
Two wheeler license online apply Maharashtra: what is different?
For a two wheeler license online apply Maharashtra request, you select Maharashtra on Sarathi and follow the same national learner and licence screens. What changes is the RTO you choose, such as Mumbai Central, Pune or Nagpur, and the test slots that office releases each week.
Choosing the right RTO
Pick the RTO that covers the address on your proof, not the office nearest your workplace. An application sent to the wrong office is usually returned, and you lose the slot.
Marathi and English names
Some Maharashtra records show a name in Marathi as well as English. Check that the English spelling on the preview matches your Aadhaar letter for letter, because that is what police and insurers read.
How do you apply for a Himachal Pradesh driving licence?
A Himachal Pradesh driving licence is applied for on Sarathi by choosing Himachal Pradesh as the state and then the learner licence service. Applicants upload documents and a signature, pay online and, as reported by the state, finish a short online safe-driving tutorial within seven days of applying.
From learner to permanent licence in HP
Guides for Himachal Pradesh say the permanent licence can be applied for between 30 and 180 days after the learner’s licence is issued. The driving test is taken at the RTO or registering and licensing authority for your area.
Hill roads and heavy classes
Himachal’s roads are steep and narrow, and drivers of buses and trucks may need a hill driving endorsement besides the heavy class. Ask the RTO whether your route needs it before you book a heavy test.
WB licence: which portal does West Bengal use?
A WB licence is applied for on the national Sarathi portal, by selecting West Bengal on the state page. You can also reach it from Parivahan Sewa through Online Services and then Driving Licence Related Services. There is no separate West Bengal licence website for new applications.
Fees in West Bengal
Third-party guides quote different West Bengal amounts, and none could be confirmed from the state transport department. Use the Rule 32 table above as the central base and read the final amount on the Sarathi payment screen.
Licence number format
A West Bengal licence number starts with WB and the two-digit RTO code, followed by the year of issue and a serial number. The format is the same 15-character pattern used across India.
West Bengal license: from learner to permanent
A West Bengal license follows the national two-step route: a learner’s licence after an online or RTO learner test, then a permanent licence after a driving test. You must hold the learner’s licence for at least 30 days before the driving test, and the learner’s licence lapses after six months.
Kolkata and district offices
Kolkata applicants choose their area office on Sarathi, while district applicants choose the office for their district. The screens and documents are the same; only the test track and slot calendar differ.
Bengali names on records
Bengali surnames are often spelt in more than one way in English, such as Chatterjee and Chattopadhyay. Pick the spelling on your Aadhaar and use it everywhere, including the guardian’s name.
Bike licence apply online West Bengal: step by step
To complete a bike licence apply online West Bengal request, select West Bengal on Sarathi, apply for a learner licence in MCWG or MCWOG, upload documents and pay, pass the learner test, and after 30 days book the driving test at your RTO.

What to carry to the test
Bring the learner licence, your original ID and address proof, the application printout and a roadworthy two-wheeler with valid papers. A helmet is compulsory during the test.
If you fail
You can book a retest after paying the retest fee shown on Sarathi. Your learner licence must still be valid on the new date.
RTO Surat driving licence: where do Surat applicants go?
For an RTO Surat driving licence, choose Gujarat on Sarathi and select the Surat RTO, code GJ-05, or Bardoli, code GJ-19, for areas in Surat East. The office you pick runs your driving test, so it must match the address on your proof.
Surat office details
No verified address or phone number for the Surat RTO is quoted here. Find the current address and timings in the Parivahan RTO directory or on the Gujarat transport department site before you travel.
Gujarati and English names
As in other states, the English spelling on your application decides what is printed. Check the preview against your Aadhaar before you pay.
What if the name on your licence is spelt wrong?
If the name on an issued licence is wrong, apply for a change or correction of particulars on Sarathi with proof of the correct name. The licence number, classes and validity stay the same, and a fresh card is printed. Before the card is issued, you can often correct the entry at the RTO.

Before you pay
The Sarathi preview screen lets you edit every field. Read it twice, ideally with your Aadhaar beside you. A mistake fixed here costs nothing.
After the card is printed
You need proof of the right name, such as Aadhaar, a passport, a marriage certificate or a gazette notice. The full method, documents and timelines are on our page on licence name change.
How do you avoid a name mismatch on the application?
Use one source for your name, normally Aadhaar, and copy it exactly: same spelling, same order and no initials unless Aadhaar uses them. Make sure your age proof and address proof show the same name. If they differ, update the proof before you apply rather than after.
Relation name and date of birth
Errors also creep into the father’s, mother’s or husband’s name and the date of birth. These are checked against your proofs, so they need the same care as your own name.
Shared phones and cyber cafes
If someone else fills the form for you, sit with them and read the preview yourself. Many wrong cards come from an agent typing the name from memory.
Which documents does each application need?
Every learner application needs proof of age, proof of address, a recent photograph, a signature scan and the Form 1 fitness declaration. A heavy or other transport class adds the Form 1A medical certificate and your existing driving licence. The driving licence stage adds the learner’s licence itself.
Two-wheeler and car applicants
Aadhaar usually works as both age and address proof. Applicants under 18 for a 50cc class add the guardian’s consent. Keep scans clear and under the size limit Sarathi shows, or the upload fails.
Heavy class applicants
Bring your current licence, the Form 1A medical, any Form 5 training certificate your state asks for and, where needed, the endorsement papers. The documents required for driving licence checklist covers each proof in detail.
Heavy vehicle license versus two-wheeler licence: what changes?
Both start with a learner’s licence on Sarathi, but a heavy vehicle class needs an existing LMV licence, a minimum age of 20, a Form 1A medical and a test on a truck or bus. A two-wheeler class needs only age 18 (or 16 for 50cc), Form 1 and a test on a bike or scooter.
| Point | Heavy vehicle class | Two-wheeler class |
|---|---|---|
| Minimum age | 20 years | 18 years, or 16 for 50cc |
| Earlier licence needed | LMV licence | None |
| Fitness proof | Form 1A medical | Form 1 self-declaration (Form 1A at 40+) |
| Test vehicle | Truck or bus of the class | Geared bike or scooter |
| Card codes | HTV, HGMV, HPMV, TRANS | MCWG, MCWOG |
Validity
A transport class is renewed on a shorter cycle than a private class, while the two-wheeler class follows the general validity rules for your age. Check the validity date for each class printed on the card.
Who can test you
The RTO’s motor vehicle inspector runs the test for both. Some states let accredited driver training centres certify heavy drivers; ask your RTO whether this applies.
How do the state routes compare?
All four states covered here use Sarathi for new learner and driving licences. What differs is the RTO you pick, the test calendar and a few local steps, such as the online tutorial reported in Himachal Pradesh. The table sums up the points confirmed for this guide.
| State or city | Where you apply | Local point to note |
|---|---|---|
| Himachal Pradesh | Sarathi, state HP | Online safe-driving tutorial within 7 days |
| West Bengal | Sarathi, state WB | No separate state portal for new licences |
| Maharashtra | Sarathi, state MH | Pick the RTO that matches your address |
| Surat, Gujarat | Sarathi, state GJ | GJ-05 Surat, GJ-19 Bardoli |
Before you travel to any RTO
Check the office address and slot on Sarathi or the state transport site on the day. Office lists and timings change, and we do not publish unverified addresses.
Can one application add several classes?
Yes. One learner application can include several classes, for example MCWG and LMV together, which saves the separate learner test fee. A heavy class is the exception: it is applied for later, once you have held the LMV licence for the period your state requires.
Separate driving tests
Each class still needs its own driving test on the right vehicle. You can book them on the same day if the RTO has slots.
Adding a class later
If you already have a licence, the new class is applied for as an addition. Sarathi pulls your existing record, so this is a good moment to confirm that the stored name is correct.
What mistakes delay a licence application?
The usual causes of delay are a name or date of birth mismatch, blurred document scans, choosing the wrong RTO, letting the learner’s licence expire before the test and bringing the wrong vehicle class to the test. Most of these are avoided by reading the Sarathi preview before payment.
Payment problems
If money leaves your account but Sarathi shows no receipt, wait for the status to update and use the payment verification option before paying again. Paying twice does not speed things up.
Missing the test slot
A missed slot usually means rebooking and, in some states, paying the retest fee. Keep the learner validity date in mind when you choose a slot.

Can I apply for a heavy vehicle license directly?
No. You first hold a driving licence for a light motor vehicle, then apply for a learner licence in the heavy class, pass a medical and a driving test, and the class is added to your existing licence.
Is a provisional license valid in India?
India does not issue a licence called provisional. The Indian equivalent is the learner's licence. A foreign provisional licence does not let you drive in India.
Which licence application form do I download?
Usually none. Sarathi fills Form 2 or Form 4 from your online entries and gives a printable copy. Some RTOs still accept paper forms at the counter.
Does a motorcycle license cover a scooter?
Guides from insurers say an MCWG licence is accepted for gearless scooters too, but the Act lists the two classes separately. If you want to be sure, apply for both classes together.
What is the youngest age for a scooter licence?
Sixteen, for a motorcycle with an engine of 50cc or less, with written consent of a parent or guardian. Other two-wheelers need you to be 18.
My name on Aadhaar is different from my school certificate. What do I do?
Make both match before you apply, usually by updating Aadhaar. The RTO may refuse an application when the name differs across your proofs.
Can I fix a spelling mistake before the licence is printed?
Before payment you can edit the entry. After payment, ask the RTO at your test visit; once the card is issued, apply for a change of particulars on Sarathi.
Does the WB licence look different from other states?
No. West Bengal issues the same Form 7 card as other states, with WB and the RTO code at the start of the licence number.
Where do I apply in Surat?
Choose Gujarat on Sarathi and select the Surat RTO (GJ-05) or Bardoli (GJ-19), based on your address. The test slot is booked at that office.
